This is a mod that changes your health bar so that it shows what abilities/items you are in danger of and will have active at your current health. 

This may not work for all resolutions. All I know is it works for my resolution (2560 x 1440)
THIS IS NOT THE BEST WAY TO DO THIS
The best way would be to modify #health_bar .ProgressBarRight so that it updates like the Heath Color Based on Percentage by Hanturaya  

The issue is I do not know how to do this XD
So we get this, a shotty version that will show the items/abilities along with a color when your health is low enough to be on the cusp of losing the effects or being in danger.

This does not change, it is not dynamic. This just takes advantage of how the loss of health effects the healthbar texture. (Weirdly this texture is also used for any progress bar so that will look a bit odd when taking a teleporter, picking up urn, ect.)

There is a version that is just colors if you prefer that. 
Per request I added a Shiv Only one. It is the same as the default except it has the Red and Black section of when you are in danger of being killed by Shiv with his upgraded execute. Its 25% of your Hp

The thresholds are:

20% Shiv's Execute Red/Black
25% Shiv's Upgraded Execute Orange
30% Spirit /Bullet Resilience Yellow
50% Frenzy Brown
50%-64% Default, no specific item Default Color
65% Enchanter's Emblem, Battle Vest, and Hollow Point  Magenta 
75% Fortitude Green

The icons are just on the image that is there for the health bar. Unfortunately the game doesn't have the image fit to the bar shape and instead has a rectangle that it grows. As you can see in this video, the Icons start centered then shifts to the left as the health bar refills.

As such I set up the icons based on when the health matters. I.E. You can just see the Enchanter's Icon right before you lose it.

Sadly I don't know enough about the hud's code to actually touch that, thus this shotty work around.
